reference / testimonial guide

I’m referring those of you I have asked to be a reference for me to this page. If you are unfamiliar with what might be asked of you in a facility Peer Evaluation, you can click on some examples BELOW.

These will come from the hospital or facility credentialing office and are normally sent out towards the end of my credentialing process. Please don’t spend a lot of time with these evaluations. They are more of a formality than determining my acceptance for the appointment. Getting them completed and returned promptly is my main request.

For dates we’ve practiced together (which would correspond to “Dates of Employment”) you may reference the Credentials Page and click on Assignment History link on the left. For facility addresses you may reference the Facilities Page and click on the facility name in the drop down menu list.

Some reference questionnaires can ask for additional comments. To that end, if you really enjoyed working with me and want to write something to help my practice, a written paragraph is better served on my Testimonal Section. You can write one on my Testimonial Page. Your testimonial on my website helps me much more than taking time to add it to the comment section on a facility Peer Evaluation. Having Peer and Professional testimonials on my website helps those visitors such as Anesthesia Groups or Facility Staff get to know me better through my colleagues who write them.
